The rehabilitation journey can be challenging enough, let alone dealing with wounds that hinder progress and increase discomfort. Whether they be surgical wounds, pressure injuries, vascular wounds, trauma wounds, or diabetic wounds, Nexus deploys innovative protocols to achieve successful outcomes.

Across the Nexus network, our wound care team is comprised of clinicians with specialty training and certifications in wound management — which means patients and residents are in great hands as we’re caring for their bodies and mending minds.

What is wound care?

Nexus’ dedicated wound care team focuses on skin integrity, prioritizing the cleanliness of acute or chronic wounds that have previously resisted healing. Within 24 hours of admission, individuals are assessed by a wound care clinician to ensure all wounds are identified in a timely manner and an individualized plan of care is initiated. Although many protocols are in place to safeguard against new wounds, if they do develop, the team is trained in early detection and a treatment plan will begin right away.
